2020 Navigating Hope Care Manager Funding

Please be advised that a Request For Funding Applications (RFA) for CY 2020 has been released for the Navigating Hope Care Manager through the Morris County Department of Human Services. The County is seeking a community partner for the Navigating Hope outreach team.

A County application must be submitted to be considered for 2020 funding. The package contains information regarding the Navigating Hope Initiative, the application process, and instructions for completing the RFA.

All agencies must submit one (1) signed original and one (1) copy of the completed application and any supporting documentation.

Provider Information Session

Morris County will be hosting a provider information session on the county funding application process. This session will be held on Friday, June 7th from 9:30 a.m. to 11:30 a.m. at the Morris County Public Safety Academy in the Auditorium.

While this session is not mandatory, potential applicants are strongly encouraged to attend in order to be fully informed of the application requirements, program and service priorities.

Morris County advisory group chairs and staff will give an overview of the county’s unified funding approach, available funding streams, service areas, and important information about the application process. In addition, there will be a question and answer period.

2020 CY Funding

Please be advised that funding applications are available through the Morris County Department of Human Services. Anticipated 2020 funding is intended for the provision of services identified within Morris County planning documents.

A Request for Funding Application (RFA) must be submitted to be considered for 2020 funding for the following:

  • Child Abuse Prevention (CAP) – County funding to support Child Abuse Prevention Programs.
  • Social Services for the Homeless – State funding for short-term assistance for individuals and families at-risk of homelessness.
  • Grant-in-Aid – County funding for the areas of mental health, addictions, aging, veterans, disabilities, youth services and general human services.

For 2020, the following funding streams are in a renewal year; however, new applications will be considered: *

  • State Community Partnership/Juvenile Justice – Community based services for at­risk juveniles or to prevent juvenile delinquency.*

*Programs currently supported by these funding streams have received 2020 renewal applications by e-mail.

Instructions

  • Separate applications must be submitted for each funding stream.
  • Applications must be submitted in a sealed envelope, typed, single-sided, on 8 ½ x 11 paper, using the correct funding application. Please write “2020 Application” on the outside of the envelope.
  • Do not send applications via fax or email.
  • Do not bind or staple the application.
  • Complete all sections of the application. Incomplete and/or late applications will not be considered.
  • Attach documentation only as specified.
  • Do not include any other reports, vouchers or other communications in the envelope.

2020 Funding Priorities


2020 Area Plan Funding

Please be advised that funding applications are available through the Morris County Purchasing Agent. Anticipated 2020 funding is intended for the provision of services identified within Morris County planning documents.

A Request for Funding Proposal (RFP) must be submitted to be considered for 2020 funding for Area Plan Contract Funding – designated to provide services in Access, Home and Community support for people age 60 years and their caregivers.

Area Plan Funding Application

All agencies must submit one (1) signed original and one (1) copy of the completed application and any supporting documentation.

Proposals shall be delivered to the Purchasing Agent either in person at the hour designated, or previous thereto, by delivery or mail, by Friday, June 28, 2019 at 10:00 a.m.

Area Plan RFP question and clarification submissions are due no later than June 13, 2019.

2020 Chapter 51 Funding

Please be advised that funding applications are available through the Morris County Department of Human Services. Anticipated 2020 funding is intended for the provision of services identified within Morris County planning documents.

A Request for Funding Proposal (RFP) must be submitted to be considered for 2020 funding for Chapter 51 Funding – state funding for substance abuse prevention, education and treatment.

Chapter 51 Funding Application

All agencies must submit one (1) signed original and one (1) copy of the completed application and any supporting documentation.

Application due date: July 12, 2019 at 2:00pm. Applications must be received by 2:00pm on the due date. Late applications will be returned unopened.
